Florida Physicians Trust (FPT) is a culmination of the hard work and cooperation between Independent Physicians across Florida and the leadership of a small dedicated group of Healthcare Professionals. This partnership was spurred on by the leadership of the Florida Medical Association as well as the Florida Osteopathic Medical Association, and their joint support of the Accountable Care Organization model as well as the shared vision-helping Physicians remain vigilant in the Independent Practice of Medicine and putting the utmost value on the Doctor-Patient Relationship.
To implement physician directed and reality based patient-centric care coordination interventions that enhances the quality of care provided to Medicare Fee-for-Service beneficiaries while at the same time combating the pronounced escalation of Medicare Program costs.
The established goals of FPT, a Medicare Accountable Care Organization (ACO), for care coordination are consistent with the contemporary movement toward reducing fragmentation and escalating costs within the health care system, and more specifically the Three Part Aim of the Medicare Shared Savings Program of “Better Health, Better Healthcare, and Reduced Costs”.
